You must bring a valid state or federal government issued photo ID and Insurance card, if you are insured.

Do I need a referral to make an appointment?

We are primary care physician's office. Most insurances do not require for you to get a referral to see a primary care physician. Some insurances require that you see your primary care physician first and get a referral to see a specialist. If Dr. Oza is covering your primary care physician, you may require to get a preauthorization from your PCP's office to see Dr. Oza.
